Introduction
Elements Compounding Pharmacy, LLC (“Elements,” “we,” or “us”) is required by law to maintain the privacy of health information (“Protected Health Information” or “PHI”) of the patients we serve and to provide this Notice of Privacy Practices explaining our legal duties and privacy practices.
PHI is information that identifies or may identify an individual patient and relates to their past, present, or future physical or mental health condition, the provision of healthcare to them, or payment for that healthcare.
How We May Use and Disclose Patient PHI
Treatment
We may use and disclose PHI to provide prescription medications and related services for patients. For example, when a prescriber submits a prescription through ElementsRx.net, we use the patient’s PHI to compound or dispense those medications and coordinate patient care with the prescriber.
Payment
We may use and disclose PHI to bill and collect payment for medications and pharmacy services.
Health Care Operations
We may use and disclose PHI for pharmacy operations including quality assessment, training, licensing, accreditation, audits, compliance, and technology operations.
Business Associates
Some functions may be performed by contractors or business associates, such as claims processors, technology vendors, delivery vendors, and other service providers. We require these entities to protect PHI in accordance with HIPAA and other applicable laws.
Regulatory and Legal Requirements
We may disclose PHI when required by law, including public health reporting, government audits, valid court orders, subpoenas, pharmacy board or DEA audits, or other legally mandated processes.
